Sharp Health Plan is different from other health plans. We are the only locally-based nonprofit commercial health plan offering plans designed by San Diegans, for San Diegans. Join our team, and you'll receive affordable plans, personal service and the kind of local attention you could only expect from your neighbor.

You can become a broker partner with Sharp Health Plan in three easy steps.

Email the required documents below to us, or fax them to 1-858-499-8246.

• Copy of your California insurance license
• Copy of your Errors & Omissions insurance certificate
• Completed W-9 form
• Covered California certification (if you are a certified agent through Covered California)
• Include your full contact information:
  • Name
  • Mailing address
  • Physical address
  • Email address
  • Phone number

We will send you a broker agreement, business associate agreement and an Addendum. Please review and sign/initial where applicable.

Return the completed and signed broker agreement and business associate agreement.
