Your care team includes your network, your plan medical group (PMG) and your primary care physician (PCP). Your network, PMG and PCP are listed on the front of your member ID card.
Your plan network is a group of doctors, hospitals and other medical service providers associated with your unique plan. We have four plan networks: Premier, Performance, Value and Choice. Each network covers specific geographic areas across San Diego and southern Riverside Counties.
A plan medical group is a designated group of doctors and hospitals that’s associated with your plan network. Your PMG is set based on who you choose as your primary care physician. You have access to the specialists and facilities, like hospitals and urgent care centers, that are affiliated with your PMG.
You only have access to one PMG at a time. Referrals and prior authorizations do not transfer between PMGs.
Your primary care physician is your personal doctor. When you need a checkup or routine care, want advice about a health problem, or get sick or hurt, contact your PCP’s office. They provide care when you need it, listen carefully and explain things in a way that is easy to understand. Your PCP also coordinates the care you receive from other providers, including specialists.
If your PCP was automatically assigned or if you need one who’s closer to home, you can change your PCP for any reason, at any time.
Log in to your Sharp Health Plan online account to choose or change your PCP. You can also search for a doctor, urgent care location, clinic or hospital online.
