1-4 subscribers enrolled: Minimum of 60% participation, excluding any valid waivers.5+ subscribers enrolled: Minimum of 25% participation, excluding any valid waivers.
Sharp Health Plan may agree to treat individuals covered by a co-employment agreement with a PEO as eligible employees of a California group for purposes of issuing a contract and enrollment.
