How long can an app access my personal health information after I grant consent?

After you give permission to share your personal health information with an app Sharp Health Plan will keep sharing your information until you tell us that you want us to stop.


How can I find our what apps that I have given permission to share my personal health information with?

Please contact Customer Care using the number on your Sharp Health Plan member ID card for assistance with identifying all of the apps that your gave permission to retrieve your personal health information.


I gave an app permission to use my health information. What can I do if I want to stop sharing my information with the app?

Please contact Customer Care using the number on your Sharp Health Plan member ID card for assistance with terminating the app’s access to your health information. This will prevent the app from continuing to pull your data from Sharp Health Plan’s system, including new data. However, the app will still have access to previously shared data. You will need to review the app’s privacy policy or contact the app’s customer service department to determine their policy for deleting data after access is terminated.


How can I correct inaccuracies in data retrieved by an app?

To make a correction to your name, date of birth, gender or address, you’ll need to make those changes through Covered California you can call 1-800-300-1506 (TTY: 711). You can also visit them online at Documents to Confirm Eligibility | Covered California™.


To make a correction to a diagnosis or procedure, you will need to contact your doctor’s office or facility where you had services and have them send us an updated claim. Sharp Health Plan cannot make those changes to your data.


Can I share my data with more than one app?

Sharp Health Plan does not limit the number of apps that you can share your personal health information with. You will use the same Personal Data Release user name and password each time.


Does Sharp have a list of recommended apps to use?

No, Sharp Health Plan does not currently have a list of recommended apps to use.


Can Sharp Health Plan get my health data from other payers or insurance companies?

No, currently Sharp Health Plan is not able to request your health data from other payers or insurance companies.


How do I share my health information with a loved one or appointed representative?

If someone helps care for you, please fill out and submit the appropriate authorization forms with both Sharp Health Plan and your plan medical group.

Sharp Health Plan follows HIPAA and state definitions regarding personal representatives. Sharp Health Plan is not allowed to share your health information with your friends or family unless they are your personal representative.

How do I change or remove a someone as a personal representative?

To change a personal representative, you will need to fill out a new form for the new personal representative. Please call Customer Care to sign and date the original form to remove your previous personal or if you have a copy of the original form you can update the form and send it back to us.

How do I file a complaint?

If you believe your health information has been shared without your permission, despite the app’s privacy policy stating it will not do so, or if you believe that some other deceptive act has taken plan, please report it to the Federal Trade Commission right away.
